Mackiewicz Lactation - Scheduling Policies

Before Your Visit

I want to ensure that your appointment goes smoothly and we have enough time to cover concerns. Prior to your initial visit, I will send you new client paperwork and consents. Please have that filled out if before our visit. It saves time so we can spend our visit hands on helping you.

Cancellation Policies

To minimize no-show cancellations, we have set up a cancellation policy. I know emergencies happen, but please cancel within 24 hours. If it is less than 24 hours please call me.

Payment Policies

I want to make sure you have all the necessary information regarding insurance and payments. Please submit your insurance card information in Intake Q paperwork so we can verify.

If you are self/cash pay, I can take cash or card.

No checks at this time.

Aetna Policies

Aetna - if you have Aetna insurance, I will verify coverage. Aetna will say that you have 6 covered visits, but sometimes you might have a portion to pay depending on your deductibles, plan, and coinsurance. I do not decide that, Aetna does. I try to bill as low as I can with still getting my fair payment. Insurance bases everything off a fee schedule and a percentage.

Travel Policies

Travel can be challenging. I may need to change appointments to fit everything in. To avoid excessive driving, I might group visits in one area. Sometimes, a home visit may not be possible, and an office visit may be arranged instead. I prefer to do home visits when requested, but there are times when it just won’t work.
